About The Practice 

The inspiration for Waypoint comes from a nautical term that I learned while growing up in Rhode Island and sailing in the summers with my family. A waypoint is a reference point that helps you know where you are and where you are going. It is often a turning point. My goal as a provider is to meet you where you are and then, through careful evaluation and observation, navigate a course of treatment that is specific to your child and family. I hope to be a turning point for your family.

The foundation of my practice is the belief that every client is unique. Each child is a unique individual, and each treatment plan needs to be tailored to that individual. Symptoms and diagnoses are only part of the equation. My approach goes beyond symptom management. It explores underlying biological, lifestyle, and environmental factors that can compromise mental health, such as inflammation, gut health, genetics, and nutrition.

Meet Carly

Carly Framptom, MSN, PMHNP-BC,

Carly Framptom, MSN, PMHNP-BC, a native of Barrington, Rhode Island, has called Paducah home since 2011. She graduated cum laude from Vanderbilt University in 2001 with a Bachelor of Science in Child Development. Carly continued her education at Vanderbilt University School of Nursing where she earned a Master of Science magna cum laude in nursing with a specialty in child/adolescent psychiatry.

After graduating in 2003, Carly has practiced as a psychiatric nurse practitioner in a variety of clinical settings including school-based therapy, inpatient psychiatric hospital, community mental center, site-based clinic at an alternative school, group private practice and solo private practice. She has also worked as an adjunct faculty member at Vanderbilt University School of Nursing and taught nursing at West Kentucky Community Technical College. Carly’s experiences and clinical abilities are wide and varied, making her well suited to meet patients ‘where they are.’ She is dedicated to a holistic and caring approach with her patients that looks beyond a list of diagnoses or symptoms.

Compassionate child and adolescent psychiatric care that includes thoughtful evaluations, medication management when appropriate, and ongoing support. Care is family-centered, collaborative, and tailored to each child’s unique needs.

Psychiatric Evaluations

Comprehensive evaluations that are integrative and root-cause focused. Rather than asking only what diagnosis fits, I will focus on why symptoms may be happening, exploring underlying contributors such as genetic factors, hormones, inflammation, gut health, trauma & stress, lifestyle factors, and coping patterns. Each evaluation results in an individualized care plan, which may include therapy, medication, nutrition, supplements, and lifestyle interventions.
Telehealth appointments
Telehealth is not offered for initial evaluations. Meeting face-to-face allows me to build rapport, observe subtle clinical cues, and gain a more complete understanding of your concerns. Telehealth is available for ongoing care once treatment has been established.
Medication management
Medication is one tool among many helpful and evidence-based approaches to treat mental health disorders. My approach is personalized and collaborative. Medications are used thoughtfully and strategically, with careful attention to how they affect the whole person. Effectiveness, side effects, and long-term goals are regularly re-assessed.
Care Coordination
Care is coordinated among families, schools, therapists, and pediatric providers to support the patient’s emotional, behavioral, and developmental health. Collaboration helps ensure consistency between medical care, therapy goals, school supports, and home strategies.
One-time psychiatric consultation
I offer a one-time consultation for diagnostic clarification and medication strategy recommendations. This service is consultative in nature and does not establish an ongoing provider-patient relationship. Medication changes are discussed as recommendations and need to be implemented by the treating provider.
Laboratory testing
When appropriate, I offer targeted laboratory testing to better understand biological factors such as genetics, hormones, nutrient status, gut & metabolic health, inflammation and food sensitivities, that may influence mental health. Testing is individualized and ordered only when results are likely to guide treatment decisions.

New Patient Process

Complete new patient inquiry form.
Callback from Carly to answer questions, provide information about initial evaluation, and schedule.
Initial evaluation includes two 1-hour appointments. First appointment is with parents only. Second appointment is with parents and child/adolescent. Treatment recommendations will be made at the second appointment.
After scheduling, you will receive an email with instructions on how to create a new patient portal account. Once inside account, you will need to complete intake forms and rating scales.
